Between the Registered Nutritional Therapist, Health Coach and Client.
PART 1
The Nutritional Therapy Descriptor
Nutritional therapy is the application of nutrition and lifestyle medicine sciences in the promotion of health, peak performance and individual care. Registered Nutritional Therapy Practitioners (NTs) assess and identify potential nutritional imbalances and understand how these may contribute to an individual’s symptoms and health concerns. This approach allows them to work with individuals to address nutritional imbalance and help support the body towards maintaining health.
Nutritional therapy is recognised as a complementary medicine and is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing. NTs consider each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a 'one size fits all' approach.
Practitioners never recommend nutritional therapy as a replacement for medical advice and always refer any client with 'red flag' signs, symptoms or injuries to their medical professional. They frequently work alongside medical professionals and will communicate with other healthcare professionals involved in the client's care to explain any nutritional therapy/exercise programme that has been provided.
Health well Nutrition requests that the Client notes the following:
- The degree of benefit obtainable from nutritional therapy may vary between clients with similar health problems and following a similar nutritional therapy.
- Nutrition and lifestyle recommendations will be tailored to support health conditions and/or health concerns identified and agreed between both parties.
- Health Well Nutrition cannot diagnose, or claim to treat, medical conditions.
- Health Well Nutrition’s recommendations are not a substitute for professional medical advice and/or treatment.
- Health Well Nutrition may recommend food supplements and/or functional testing (hormone, stool, blood, genetic etc.) as part of your nutritional therapy programme. These will be charged outside of the programme fee unless stated otherwise.
The Client understands and agrees to the following:
- I am responsible for contacting my GP about any health concerns.
- If I am receiving treatment from my GP, or any other medical provider, I should tell him/her about any nutritional recommendations provided by Health Well Nutrition. This is necessary because of any possible interaction between medication and the nutritional programme.
- It is important that I inform Health well Nutrition about any medical diagnosis, medication, herbal medicine or dietary supplements I am taking, as this may affect the nutritional programme.
- If I am unclear about the agreed nutritional therapy programme, supplement doses/time period, I should contact Health Well Nutrition promptly for clarification.
- I understand that the recommendations are personal to me and may not be appropriate for others.
- I must contact Health Well Nutrition should I wish to continue any specified supplement programme for longer than the originally agreed period, to avoid any potential adverse reactions.
- If I experience an adverse reaction from any supplements recommended to me, I must discontinue these immediately, inform Health Well Nutrition and contact a doctor if I feel like this is necessary.
- I will inform Health Well Nutrition of any illness that might be aggravated by eating certain foods.
- Recording consultations using any form of electronic media is not allowed without the permission of both me and Health Well Nutrition.
- I understand that all content provided to me belongs to Health Well Nutrition, and I will not reproduce or share any of such content with other parties.
Confidentiality and Data Protection
Health Well Nutrition is registered with the Information Commissioner´s Office (ICO), and will keep your personal information confidential and secure following GDPR guidelines. Health Well Nutrition uses Practice Better as a client management system, which is GDPR compliant. All your details will be stored safely within Practice Better, unavailable for anyone to access, apart from Health Well Nutrition. Your information will not be shared with third parties without your consent. However, if Health Well Nutrition believes there is a risk of significant harm to yourself or another person, they may pass the information onto an appropriate authority using the legal basis of duty of care.
Health Well Nutrition can use the personal information you provide to:
- Provide the services
- Process your payment for such services
- Add you to the newsletter emailing list, which provides information relevant to your health concerns, and any similar services Health Well Nutrition provides that may be of interest to you. You can stop receiving these communications at any time by selecting the ‘Unsubscribe’ button at the bottom of such emails.
- Present a case study for educational purposes of other health professionals, although your identity will always be kept completely anonymous.
- Refer you to a different practitioner/GP or contact your Emergency Contact if necessary.
Programme Terms of Engagement
PART 2
Depending on the programme you have chosen, you will have regular contact time with Health Well Nutrition, either face to face or over Zoom. In all programmes you will receive personalised nutrition, exercise and lifestyle advice sent to you.
Payment and Booking:
- Full payment is due before your first appointment. This can be done by following the payment link on the Health Well Nutrition website or alternatively with manual bank transfer. Health Well Nutrition reserves the right to cancel your appointment without successful payment.
- Once payment has been made, it is advised to pre-book your sessions as far in advance as possible to avoid disappointment of someone else booking your preferred slots. You can do this by contacting Maria at maria@healthwellnutrition.co.uk.
- Please be available for your appointments at the agreed start time. If you are late, the duration of the service may unavoidably be reduced.
- It is your responsibility to book your sessions to match your selected programme time frame.
Cancellations/rescheduling:
- A minimum 24-hour notice is necessary to reschedule an appointment. The session will be forfeited in cases of no-shows or if notice is given any later than 24 hours.
- It is your responsibility to ensure you have a working internet connection for online appointments. If you are unable to connect as scheduled, the appointment might be considered a no-show and charged in full.
- For optimal results, your programme is designed to be completed within the given time frames (6 months, 4 months, 3 months and 4 weeks). If this is not possible, then all programmes must be completed within 10 months for 6 month programmes, 8 months for 4 month programmes, 6 months for 3 month programmes and 3 months for 4 week programmes from the day of purchase (by special arrangement only). Any outstanding sessions will be forfeited after this.
- Health Well Nutrition reserves the right to amend any consultation upon 24 hours notice.
- If you have commenced a programme and wish to cancel prior to the end of all scheduled services offered within the programme, you shall be obligated to pay all fees accrued prior to the effectiveness of your cancellation. Health Well Nutrition reserves the right to include fees for all preparatory work and research undertaken, in addition to the scheduled contact dates for consultations and calls.
- Health Well Nutrition reserves the right to decline to provide the Services to you, for any reason, and will refund any amounts pre-paid by you for services not yet received.
Third parties:
Nutrition professionals are automatically set up to receive commission on supplements/clinical testing. However, Health Well Nutrition has chosen to hand most of this percentage to their clients, giving you 15% off supplements from the Natural Dispensary. Discounts on clinical testing may vary depending on the testing company we are purchasing your tests from.
By signing these Terms & Conditions with Health Well Nutrition, I confirm that I have read, understood and agree with all Terms & Conditions listed in this document.
